One-Week Regional Nodal Radiotherapy Maintains Quality of Life in Breast Cancer Patients
Regional nodal irradiation (RNI) remains a key component of treatment for selected patients with early-stage breast cancer, but emerging evidence is helping clinicians better identify who truly benefits—and who may safely avoid additional radiation.
RNI targets lymph node regions beyond the breast, including the axillary, supraclavicular, and internal mammary nodes, to reduce recurrence and improve long-term outcomes. Historically recommended for node-positive disease, its role is becoming increasingly individualized as systemic therapies and surgical approaches evolve.
One of the most practice-changing datasets comes from the NRG-NSABP B-51/RTOG 1304 trial, which evaluated patients who initially had node-positive breast cancer but achieved pathologic node-negative status (ypN0) after neoadjuvant chemotherapy. At a median follow-up of 59.5 months, adding RNI did not significantly improve invasive recurrence-free survival or other oncologic outcomes, suggesting that carefully selected patients may safely omit nodal irradiation and avoid unnecessary toxicity.
Long-term evidence also highlights the importance of balancing benefit against late effects. A 20-year analysis of the EORTC 22922/10925 trial found that internal mammary and medial supraclavicular (IM-MS) irradiation reduced breast cancer mortality from 14.2% to 10.0% in node-negative patients but did not improve overall survival (69.0% vs. 68.4%). The study also identified a higher rate of non-breast cancer mortality after prolonged follow-up, underscoring the need for careful patient selection and modern organ-sparing radiation techniques.
Experts emphasize that decisions regarding nodal radiotherapy should consider recurrence risk, response to systemic therapy, potential toxicity—including lymphedema and cardiopulmonary exposure—and patient preferences. As evidence continues to support treatment de-escalation in appropriately selected patients, RNI is shifting from a one-size-fits-all approach to a more personalized strategy.
