Transperineal Focal Laser Ablation Shows Promise in Treating Localized Prostate Cancer

An international registry study assessed one-year outcomes in 195 patients who underwent transperineal focal laser ablation for localized prostate cancer at four centers between May 2018 and June 2023.

According to RSNA's September 2026 report, 83% required no additional whole-gland therapy at one year. Biopsy-confirmed clinically significant recurrence occurred in 17%, underscoring the need for surveillance and, in some cases, further treatment.

The procedure uses ultrasound guidance to place laser fibers through the perineum. Follow-up combined PSA testing, MRI and biopsy; MRI findings helped identify patients requiring further assessment.

For radiation oncology teams discussing local treatment options, the study offers information on an alternative approach for selected patients. It does not establish equivalence or superiority to radiotherapy or prostatectomy. Patient selection, accurate tumor delineation and standardized follow-up remain important, particularly because MRI can underestimate tumor extent.
