Recent findings presented at the American Society for Radiation Oncology (ASTRO) meeting have shed light on the potential of MR-guided adaptive radiotherapy using Elekta Unity for patients with localized pancreatic cancer. Conducted at Memorial Sloan Kettering Cancer Center, the study monitored outcomes for patients ineligible for surgical resection, a demographic traditionally associated with poor prognoses.

Localized pancreatic cancer poses substantial treatment challenges due to its proximity to vital organs. Therefore, securing precise radiotherapy is crucial. Elekta Unity, an MR-linear accelerator (MR-Linac), enables the adaptation of radiotherapy based on real-time MR imaging. This capability allows clinicians to adjust treatment delivery in response to anatomical changes, enhancing precision and potentially improving outcomes.

In this study, 123 patients received MR-guided adaptive radiotherapy from May 2020 to December 2024 via Elekta Unity. Over two weeks, patients underwent five treatment sessions accompanied by daily adaptation tailored to their specific anatomy. With a median follow-up surpassing two years, researchers noted an overall survival rate of 37.4% at two years and 26.3% at three years. Furthermore, local tumor control was sustained, with 76.8% of patients free from local progression over the same period.

Dr. Marsha Reyngold from Memorial Sloan Kettering, the study's primary author, emphasized the significance of radiotherapy in achieving long-term survival outcomes for those unable to undergo surgery. "Our approach using Elekta Unity to deliver high-dose ablative radiotherapy was associated with durable local control and very promising long-term survival outcomes," she stated, indicating the promise of continued research in this area.

The findings also spotlight the critical role of advanced technology in enabling precision and personalization in treatment. Elekta has announced further advancements with the development of Unity Pro, an evolution of its MR-guided adaptive platform. The new system aims to facilitate faster and more practical adaptive radiotherapy through AI-assisted contouring, accelerated planning, and enhanced collaboration capabilities. This evolution represents Elekta’s commitment to simplifying workflows and broadening access to adaptive radiotherapy in clinical settings.

These results are an encouraging step forward for treating localized pancreatic cancer, suggesting that MR-guided radiotherapy could prove invaluable in a broader therapeutic arsenal, particularly for patients with limited alternative options.
